(2) Complaint: Disagreement not related to products and/or services; or discomfort or dissatisfaction with customer service.

* Filing this complaint does not exclude the use of other means of dispute resolution and is not a prerequisite for submitting a claim to Indecopi.

* The provider must respond to the complaint within a maximum period of fifteen (15) calendar days, which may be extended for an additional fifteen days.

* By signing this document, the customer authorizes follow-up contact after the complaint process to assess the quality and satisfaction of the service received.
